Form 6-1
This is the newly approved version of the Application for Approval Form 6-1 that must be used by all non-government service providers seeking eligibility to receive recurrent funding from DSQ. Please note if you have already commenced completion of the original version of this form DSQ will accept this version until the 15 February 2008. Any applications on the original version of Form 6-1 signed by the applicant after 15 February 2008 will not be accepted by the department.
