From 3 December 2007, the Application for Support is the only form required for a person with a disability to register or apply for Disability Services Queensland specialist disability services. This form replaces the Disability Services Queensland registration form and all other program-specific applications for Disability Services Queensland services and funding programs. It only has to be submitted once.
Registering with Disability Services Queensland requires completion of only a limited number of questions in the Application for Support, as instructed in the form. Applying for specialist disability services and funding programs requires completion of the full Application for Support.
The full Application for Support collects information that Disability Services Queensland requires to determine a person’s relative need for specialist disability services, and to identify the support options available to meet the needs of the applicant and their family (where relevant).
What are specialist disability services?
Disability Services Queensland funds and provides specialist disability services.
Specialist disability services are services and initiatives specially designed to meet the needs of people with disabilities. An Application for Support is required for the following specialist disability supports:
- accommodation support
- community support
- community access, and
- respite.
Who can apply for support?
To be eligible for specialist disability services, a person must have a disability as defined by the Disability Services Act 2006 (Qld) and meet the Queensland and Australian residency criteria. This is detailed on pages six and seven of the guide to Completing the Application for Support.
What if I have previously registered or applied for Disability Services Queensland services or funding?
If you have previously registered with Disability Services Queensland, but have not submitted at least one program specific application form, a full Application for Support will need to be submitted if you wish to be considered for Disability Services Queensland specialist disability services.
If you have previously registered with Disability Services Queensland and have also submitted at least one program specific application form, in most cases you will not need to submit an Application for Support as your original application remains current.
If however, you have experienced a significant change in circumstances (such as moving out of the family home or existing school) since previously applying for support, contact your local Disability Services Queensland Regional Office for advice.
What if my circumstances have changed?
If your circumstances change after an application has been submitted, a Disability Services Queensland Application Update Form should be completed. Alternatively, you may wish to submit a new Disability Services Queensland Application for Support if your circumstances have changed significantly.
Where can I get a Disability Services Queensland Application for Support form?
Copies of the Application for Support are available at any Disability Services Queensland service centre or on this website.
Who can complete the Disability Services Queensland Application for Support?
The Disability Services Queensland Application for Support can be completed by a person with a disability over the age of 18, or by a family member (if the applicant is under 18 or unable to apply on their own behalf). A carer, advocate or service provider may also complete an application form, if requested by the applicant. If the applicant or their family is unable to complete the form without assistance from Disability Services Queensland, contact a local Disability Services Queensland service centre through the Disability Information Service on 1800 177 120.
What happens to the Application for Support form once Disability Services Queensland receives it?
Disability Services Queensland checks that all Application for Support forms have the necessary questions answered, and that the required documentation is attached. If additional information is required, the applicant or their primary contact person will be notified.
As demand for services continues to exceed supports available, applications for specialist disability services are reviewed by Disability Services Queensland to determine the applicant’s relative priority needs for services. An applicant will be advised of the outcome of this determination process.
An Application for Support does not guarantee access to Disability Services Queensland provided or funded specialist disability services. Support is provided according to available resources.
What happens if I am not happy with the outcome of the application process?
Disability Services Queensland acknowledges the right of people to make a complaint or discuss a concern about a process or decision. The Disability Services Queensland service centre where the application for support was lodged should be contacted for this purpose.

What if I need more information or do not understand the questions in the Application for Support?

If additional information is still required after referring to the relevant section of the guide, please contact your nearest Disability Services Queensland service centre through the Disability Information Service on 1800 177 120.
Where do I lodge the application form?
The Application for Support can be posted to the nearest Disability Services Queensland regional office, or hand delivered to a Disability Services Queensland service centre located throughout Queensland.
