Word format versions of the forms are available for download, should you wish to complete the form electronically. Completed Word format versions of the form should be printed and a signed hard copy posted to a Disability Services Queensland regional office or hand delivered to a Disability Service Queensland service centre. Electronic copies of the forms can not be accepted.

Application for Support (for new applicants)

Download this form if you are applying for support from Disability Services Queensland for the first time, or if you have previously registered but have not submitted at least one program-specific application form.

Application Update Form (for current applicants)

Download this form if you have already applied using a program specific application form, or have applied using the single Application for Support. Contact Disability Services Queensland before completing an Application Update Form if your circumstances have changed significantly since you previously applied.
