The Disability Services Act 2006 outlines the requirements for the criminal history screening for people engaged by a funded non-government organisation.

In order to carry out the screening process, Disability Services Queensland will collect information through a range of forms. The Act outlines a number of approved forms for the collection of information to be used for a number of different purposes related to criminal history.

Application for criminal history screening (prescribed notice) (Form 10-1)

Used each time an application is made by a non-government service provider funded by Disability Services Queensland for a criminal history check on a person engaged or about to be engaged by the organisation.

Application for criminal history screening change of details (Form 10-2)

Used by a person engaged or seeking to be engaged by a non-government service provider funded by Disability Services Queensland (DSQ) if their name or contact details change or their engagement with that funded non-government service provider ends before they have been issued with a DSQ positive notice or DSQ negative notice.

Application for replacement (positive notice card) (Form 10-3)

For use by a person engaged by a non-government service provider funded by Disability Services Queensland whose positive notice or positive notice card has been lost or stolen.

Change of details of positive notice holder (Form 10-4)

For use by a person holding a positive notice or a positive notice card who changes their name, starts to use a different name or changes their contact details.

Application for cancellation of negative notice and issue of positive notice (Form 10-5)

For use by a person seeking to have a negative notice cancelled and to be issued with a positive notice and a positive notice card.

Application for information from register of engaged persons (Form 10-6)

For use by a non-government service provider funded by Disability Services Queensland requiring information about the current status of an application for criminal history screening or any positive notice or negative notice issued.
